Do you take my insurance?
Grassroots is a cash pay practice, meaning that we do not network with any private insurance companies or Medicare/Medicaid.
Our rates are straight forward and due at the time of service. You will not receive any surprises, such as a hefty bill in the mail.
At our clinic, a Doctor of Physical Therapy will work with you 1-on-1 for the entirety of each visit.
We respect your investment in your body. Our focus is on helping you achieve your goals, educating you, and putting you at the helm of your medical care.
Some private insurance plans may reimburse you for our services at an ‘out-of-network’ rate. This does not apply if you are using Medicare, Workers’ Compensation, Select Health in Utah, or auto insurance.
We recommend checking with your insurance company regarding the specifics of your ‘out-of-network’ benefits. We will happily generate a detailed invoice for you to submit to your insurance company for reimbursement.

What is your cancellation policy?
We have a 24-hour cancellation policy. No-shows and cancellations with less than 24 hours notice will be billed in full, except in cases of sudden illness or emergency.
All payments are due at the time of service. We accept cash, all major credit cards, and debit cards (including HSA and FSA).
